PRIVACY & INQUIRIES
How to contact us safely.
We welcome general questions about virtual care coordination, family support, referrals, and potential partnerships. Our public website form and ordinary email are not intended for sending private medical information.
Do not include patient names with medical details, diagnoses, medications, insurance or government ID numbers, clinical records, or other sensitive health information in the public contact form or ordinary email. For an urgent medical concern, contact the appropriate clinician or emergency service; this website is not an emergency service.
What you may share in a general inquiry
Your name, organization, work contact details, and a brief description of the type of service or partnership you are interested in—without patient-specific information.
How general inquiries are handled
Information submitted through the website contact form is transmitted through our website hosting and form processing service (Netlify) and delivered for business follow-up. We use the contact information you provide to respond to your inquiry and discuss services or potential partnerships. Website and email providers may process and retain submitted information under their own service arrangements. We do not describe this public form or ordinary email as a secure channel for patient records.
Patient referrals and protected information
Before exchanging identifiable patient information, our organization and the referring partner must establish an appropriate secure referral and communication process, including applicable agreements, authorization or other permitted basis for sharing, access controls, and any required privacy safeguards. A general inquiry does not create a patient-provider relationship or a service agreement.
Access, correction, and questions
For questions about an inquiry you sent, or to request correction or deletion where applicable, contact us by phone at 413-450-1500 or email nnabitaka@wmhomenurses.com using only general information. Do not include medical details in your request by ordinary email. Requests are reviewed in light of applicable obligations and service-provider retention practices.
